Responsibilities

  • Assesses clients and documents pertinent information including Triage, Medical/Behavioral Health assessments and medication administration utilizing appropriate assessment tools. Accurately record all medication brought in by patient on medication reconciliation/storage form.
  • Plans client treatment as part of the Multidisciplinary Clinical team.
  • Provides education concerning Psychiatric diagnosis, treatment options and psychopharmacology to clients, families and all others involved in the clients’s care.
  • Intervenes with appropriate verbal interventions to diffuse out of control or potentially dangerous behaviors. Demonstrates ability to effectively use therapeutic option theory and technique and can describe and demonstrate their roles and responsibilities.
  • Demonstrates a thorough knowledge of Seclusion/Restraint standards, guidelines, techniques; Initiate documentation and debriefing with consumers for any seclusion/restraint incidents as required.
  • Accurately transcribes Provider orders and administers medications as prescribed.  Performs chart checks and signs off as a second noted on medications prescribed. Draw labs and provide wound care as ordered by Providers when required.
  • Completes nurse to nurse at discharge or when accepting patient from another facility.

Minimum Qualifications:

  • Graduated from an accredited Nursing program with licensure by the Arizona Board of Nursing.
  • Unrestricted license as Registered Professional Nurse through the Arizona State Board of Nursing
  • At least 2 years of psychiatric, behavioral health, or relevant experience
  • Valid Arizona Level I Fingerprint Clearance Card

Responsibilities

  • Conducts security screening and inventories property of individuals upon admission to the facility. Provides orientation to the unit.. Educates patients on their rights under ADHS regulations, grievance process, provides expectations, and explains rules and regulations.
  • Intervenes with escalated individuals using therapeutic techniques for de-escalation and performs restraints/seclusions when needed to ensure the individual’s safety and the safety of others. Completes 1-to-1 on a patient in seclusion or mechanical restraints Q15.
  • Lifts, moves and transports individuals, using proper body mechanics or lifting devices for accident prevention. Assists individuals with activities of daily living to include eating, bathing, dressing, toileting and transferring.
  • Conducts and documents individual’s vitals and reports all questionable results to the RN/Physician attending the individual. 
  • Attends court ordered hearings when subpoenaed. Must be willing to be a witness on a petition and available to provide testimony at court ordered hearing.
  • Monitors and documents patients Q15.
  • Responsible for maintaining and ensuring the cleanliness of the facility and designated assigned unit. Performs light cleaning of interior center, collects soiled linen, and launders individuals clothing when needed.

Qualifications

Minimum Qualifications:

  • High school diploma or equivalent
  • At least 2 years of behavioral health experience
  • Must be able to perform Safe Clinch patient restraint techniques.

Preferred Qualifications:

  • Certified training (Safe Clinch, CPR First Aid, therapeutic options, etc.)
  • Completion of CNA or Patient Care Tech program

Full Time 40 Hours per Week

Under the supervision of the Chief Nursing Officer, the Clinical Practice Educator and Quality Improvement position facilitates the education of employees to improve patient care outcomes.  Responsibilities include onboarding and orientation education, assessment of competence, continuing education needs, development of programs to assist in professional growth and leading or participating in inter-professional research, evidence based practice and quality improvement projects.  Responsible for the implementation and compliance of Relias throughout the facility.  This position works collaboratively with leadership and independently manages assignments, seeking assistance when needed.  This position facilitates positive environments for clinical learning and professional development.

Qualifications:

Must have knowledge of community based behavioral health services.  Must have knowledge of the spectrum of mental illnesses, DSM-5 codes and descriptions, psychotropic medications and crisis intervention.  Must be energetic, flexible, creative, innovative individual who likes to be a part of a team but can also work independently.

Requires a Bachelor’s degree or higher in Nursing.  Prefer Master’s degree in Nursing.  Requires a minimum of three (3) years of psychiatric RN clinical practice that demonstrates a strong understanding of the required knowledge, skills, and abilities.  Must possess a current, valid AZ RN license.  Must be in good standing with the Arizona Board of Nursing.  Requires BLS issued CPR certification.  Requires Level 1 Fingerprint Clearance Card.  Demonstrates the ability to lead others; work autonomously and in group settings; be highly motivated with a strong incentive to succeed. Demonstrates flexibility and ability to adjust schedule to meet organizational needs; manage time and stress effectively. Demonstrates effective problem solving, prioritization, and decision-making skills. Requires excellent written and verbal communication skills. Role model for professional behavior and critical thinking. Requires ability to work with culturally diverse people. Requires the ability to read, write and speak effectively in English
